An employee was attempting to move a steel form panel with a crane when the spreader beam became tangled with a rebar cage. As the load began lifting, the tension from the chains making contact with the rebar cage allowed the load to swing toward the employee, trapping him between the steel form and a solid staircasing and resulting in an injury to his left leg.

An employee was standing on a six foot folding step ladder assembling wooden forms. The plywood that the ladder was on shifted and the employee fell 18 feet off the side of the building resulting in a back injury.
